Please make sure to complete this form at the time that you reserve your event space in facility scheduler but no later than one week prior to your event. (We would rather be prepared for providing childcare and have you cancel last minute than struggle to find care for your event last minute.) Please allow 48 business hours for a confirmation from the children's ministry department. If you do not complete this form do not assume you will have childcare for your event.
